Job summary

Edgefield Post Acute, a respected skilled nursing and rehab facility in Edgefield, SC, seeks an experienced RN or LPN Wound Nurse to join a compassionate interdisciplinary team. You will assess and treat wounds, collaborate on individualized care plans, and educate residents and families on prevention and care.

The role requires not entry-level experience, current RN or LPN licensure, and a minimum of 2 years in nursing, with strong assessment and documentation skills.

Qualifications

• Not entry-level; must have wound care experience.
• Must have an active RN or LPN license in good standing.
• Minimum of 2 years of nursing experience.
• Excellent assessment, documentation, and communication skills.
• Strong clinical judgment and ability to work with interdisciplinary team.

Responsibilities

• Assess, manage, and treat wounds, pressure injuries, skin tears, ulcers, and other conditions.
• Complete skin assessments and quarterly dermatologic evaluations.
• Develop individualized care plans with physicians and the care team.
• Provide accurate written and verbal reports on resident progress and wound healing.
• Monitor wound healing, update care plans, and adjust treatments as needed.
• Educate residents and families on wound prevention and treatment plans.
• Participate in staff education and in-service wound care training.
• Assist with wound care supplies and equipment inventory.
• Maintain complete documentation compliant with HIPAA and regulations.
