McKendree Post Acute and Rehab in Hermitage, TN is hiring a Wound Care Nurse (LPN or RN) to assess, treat, and manage wounds in residents of a skilled nursing facility. The role collaborates with interdisciplinary teams to ensure high-quality care and regulatory compliance.

Qualifications

• Active LPN or RN license in good standing.
• Wound care experience preferred (SNF or long-term care a plus).
• Wound Care Certification (WCC) preferred or willingness to obtain.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and teamwork abilities.

Responsibilities

• Perform comprehensive wound assessments and develop individualized care plans.
• Provide and oversee wound treatments, including dressing changes and infection prevention.
• Monitor healing progress and adjust care plans as needed.
• Collaborate with physicians, nursing staff, and therapy teams.
• Ensure accurate and timely documentation of all wound care services.
• Educate staff on proper wound care techniques and prevention strategies.
• Maintain compliance with all state, federal, and facility guidelines.
