Job summary

National Health Care Associates is seeking a Wound Care Nurse in West Hartford, CT to assess, treat, and monitor wound conditions as part of an interdisciplinary team. You will develop care plans and provide direct interventions to improve healing and residents’ quality of life.

The role emphasizes collaboration with physicians and staff, ongoing education, and ensuring regulatory compliance. Strong nursing license and wound care experience are essential.

Qualifications

• Valid state RN nursing license.
• Degree in nursing, epidemiology, biology, or microbiology preferred.
• 2+ years in wound care management, preferably in long-term care.
• Certified Wound Care Nurse (CWCN) certification preferred.
• Strong clinical assessment and communication skills.

Responsibilities

• Provide oversight and expertise of wound care and prevention program for residents and staff.
• Conduct comprehensive wound assessments and develop individualized care plans for patients exhibiting skin complications.
• Provide direct wound care interventions, including dressing changes, debridement, and wound irrigation.
• Review, update and direct the implementation of wound care & prevention policies and procedures.
• Monitor wound healing progress and evaluate the effectiveness of treatment modalities.
• Educate residents, families, and staff on wound care protocols and prevention strategies.
• Collaborate with physicians, wound care specialists, and other healthcare professionals to coordinate care.
• Maintain accurate documentation of wound assessments, interventions, and outcomes for regulartory compliance and reimbursement
