Job summary

Integrated Wound Care is seeking a compassionate Nurse Practitioner to join our focused wound care team. You will assess wounds, design treatment plans, document thoroughly, and educate patients and families to improve outcomes in skilled nursing and rehabilitation facilities.

The role requires CT travel, independence, and collaboration with a clinical team. A valid NP license and at least 1 year in wound care are essential; full benefits and support are provided.

• Travel throughout Connecticut would be required.

Responsibilities

• Conduct wound assessments and provide personalized treatment plans for patients.
• Work collaboratively with a team of healthcare professionals to ensure optimal patient care.
• Complete documentation accurately and in a timely manner.
• Educate patients and their families on wound care, prevention, and management.
• Monitor patient progress and adjust treatment plans accordingly.
• May perform Ultrasound Mist therapy 2‑3x weekly (training provided).
• Administrative work from home duties; ratio of admin to clinical work varies by need.
• Full-time travel within CT; assist in obtaining licenses for full-time travelers.

As a Nurse Practitioner with Integrated Wound Care, you will play an instrumental role in making a positive impact on the lives of our patients.

Responsibilities

• Conduct wound assessments and provide personalized treatment plans for patients
• Work collaboratively with a team of healthcare professionals to ensure optimal patient care

• Educate patients and their families on wound care, prevention, and management
• Monitor patient progress and adjust treatment plans accordingly
• May perform Ultrasound Mist therapy 2‑3x weekly (experience in Ultrasound not needed, will train)
• Administrative work from home duties
• Ratio of administrative and clinical duties will vary based on need
• Part time may expect some local travel up to 2‑3 hours
• Full time may expect frequent travel mostly within CT
  • for fulltime travelers, we will assist in obtaining other state licenses
