Job summary

Qualifications

• Licensed practical/vocational nurse with active state license.
• Minimum 1 year experience in long-term care wound care.
• Minimum 2 years nursing experience in skilled nursing facility preferred.
• Current BLS/CPR certification as required.

Responsibilities

• Perform resident skin assessments, obtain treatment orders, and document appropriately.
• Monitor wound healing and adjust orders as needed.
• Maintain records of pressure ulcers and skin assessments.
• Educate new staff on Skin Practice Guide and wound care resources.
• Coordinate supplies with central supply and assist director with case reviews.
• Provide monthly, quarterly, and annual QA reports.
