Outfield Healthcare Partners in Independence, Missouri is seeking a dedicated Wound Care Nurse (RN or LPN) to lead skin integrity and wound management within our skilled nursing team. This role focuses on promoting healing, preventing breakdown, and ensuring regulatory compliance while supporting residents and staff.

Qualifications

• Graduate of an accredited RN or LPN program.
• Active nursing license in Missouri required.
• Minimum 1 year wound care experience in long-term care preferred.
• 2+ years skilled nursing facility experience preferred.
• Current BLS/CPR certification.
• Strong assessment, documentation, and communication skills.
• Knowledge of skin integrity programs, prevention strategies, and regulatory requirements.

Responsibilities

• Perform comprehensive skin and wound assessments.
• Carry out wound care treatment orders and monitor progress.
• Document assessments, treatments, and outcomes accurately.
• Track pressure ulcers and skin changes with weekly measurements.
• Ensure Braden risk assessments are completed.
• Educate staff on wound care protocols and best practices.
• Coordinate wound care supplies with central supply.
• Support QAPI initiatives and stay updated on standards.
