Job summary

Life Tech is seeking a Wound Care Nurse Practitioner to join our team delivering specialized wound care in patients' homes, nursing facilities, and assisted living facilities across Livingston and surrounding areas.

The ideal clinician works autonomously, prioritizes patient-centered care, and focuses on improving outcomes for acute and chronic wounds. NJ NP licensure, WOCN/WCC certification preferred, and at least 1 year of wound care experience are sought.

Qualifications

• New Jersey NP license required.
• WOCN or WCC certification preferred.
• Minimum 1 year wound care experience (home health or facility-based a plus).
• Excellent communication and documentation skills.
• Valid driver’s license and reliable transportation.

Responsibilities

• Perform comprehensive wound assessments and develop individualized treatment plans
• Provide advanced wound care, including debridement, dressing changes, and infection management
• Collaborate with physicians, caregivers, and facility staff to coordinate care
• Document visits and treatment plans accurately in EMR systems
• Educate patients and caregivers on wound prevention and care
• Travel to patient locations within Livingston and nearby communities

Role Overview
