Job summary

Plazawesthrc is seeking a wound care nurse to coordinate the center's wound team and oversee skin management program implementation. You will lead rounds, review interdisciplinary care plans, and document orders and outcomes.

Preferred candidates hold wound nurse certification and infection preventionist certification, with at least one year of nursing management experience. This full-time role emphasizes data gathering and staff education in pressure ulcer prevention.

Qualifications

• Current RN or LPN license in state of practice.
• Wound nurse certification and infection preventionist certification preferred.
• One year of nursing management experience preferred.

Responsibilities

• Coordinate and lead weekly wound team rounds and care plan reviews.
• Identify and maintain methods to gather pressure ulcer and skin alteration information.
• Provide pressure ulcer data analyses to nursing staff and practitioners.

Job description

As the wound care nurse you are designated to act as the coordinator of the center’s wound team to oversee the general application of the skin management program. The following activities are performed as directed or clinically indicated.

GENERAL DUTIES

·Identify and maintain effective methods to gather pressure ulcer and skin alteration related information

·Coordinate and lead weekly wound team rounds and processes, including rounds, measurements, interdisciplinary care plan review, treatment recommendations, physician orders, and required documentation

·Collect, analyze and provide pressure ulcer information to nursing staff and other healthcare practitioners

QUALIFICATIONS

·Currently licensed as RN or LPN in state

·Prefer wound nurse certified and infection preventionist certified

·One-year prior nursing management experience preferred.
