Wound Care Nurse - RN or LPN

Position Summary: Responsible for maintaining clinical competency, applying nursing knowledge and skills, leadership, and communication skills. Uses the nursing process for assessment, planning, and care implementation. Demonstrates organizational ability to prioritize patient care needs. Complies with current standards of practice and regulations, and engages in continuous quality improvement and problem-solving.

Specific Duties and Responsibilities

1. Collaborate with the interdisciplinary care team for accurate patient assessment and development/revision of individualized wound care plans.
2. Maintain open communication with patients and families regarding wound status, encouraging participation in decision-making.
3. Assess all patients for wounds upon admission or return within 24 to 72 hours.
4. Visually inspect patient skin weekly, delegate inspections when indicated, and report issues promptly.
5. Review and update wound care orders, report wound status, and recommend next steps for healing.
6. Update patients and families on wound healing progress.
7. Perform weekly wound measurements, record in the EHR, and report as required.
8. Participate in Quality Assurance/Performance Improvement activities related to wound care and pressure ulcer prevention.
9. Provide in-services on wound care and pressure ulcer prevention, ensuring staff knowledge of pressure relief and positioning aids.
10. Assist with medication administration, treatments, and patient admissions as needed.
11. Supervise LPNs and other nursing staff, complete evaluations, and address personnel issues promptly.
12. Assist physicians during rounds and support patient dignity and safety.
13. Aid patients in achieving optimal self-care.
14. Perform other duties as assigned.

Education and Training

• Graduate from an accredited nursing school.
• Maintain current licensure as an LPN or RN in the state of employment.
• Wound Care Certification preferred.
