Wound Nurse

RN/LPN

Responsible for implementing the Wound Prevention and Treatment Program in the facility. Collaborates with the Wound Care Team to determine appropriate wound prevention and implements treatment modalities as assigned.

Duties and Responsibilities

• Assess skin condition on all new admissions using standardized assessment tools.
• Obtain physicians’ orders for treatment modalities in collaboration with the Wound Care Team.
• Implement treatment plans as assigned by the Director of Nursing.
• Document all treatments accurately, review previous documentation, and ensure consistency.
• Complete weekly wound summaries.
• Respect residents' rights regarding care decisions, consent, and refusals. Consult with the Unit Manager and Social Worker as needed.

Experience

• Graduate of an accredited School of Nursing.
• RN or LPN license.
• Minimum one year of experience in wound care.
• Wound Certification is a plus.
