Job summary

Paradigm Healthcare in Katy, TX seeks an experienced Wound Treatment Nurse to deliver high-quality nursing care within a skilled nursing facility. You will assess and manage wounds, plan and direct individualized care, and collaborate with physicians, residents, families, and staff to ensure optimal wellbeing.

The role requires an active Texas RN or LVN license, 2+ years in long-term care preferred, and strong communication skills.

Qualifications

  • Must hold an active RN or LVN license in the state of Texas.
  • Experience in long-term care nursing is preferred.
  • Strong clinical assessment and wound care skills required.

Responsibilities

  • Provide direct nursing care in accordance with state regulations and facility policies.
  • Develop and direct the plan of care for residents based on physician orders and care guidelines.
  • Conduct rounds, document care, and communicate with residents, families, and staff.
  • Provide education to residents, families, and caregivers as needed.
  • Perform CPR and provide emergency support per physician orders and facility policy.

Calling all candidates who want to be part of a fun and growing team!We are a skilled nursing facility seeking an experienced, compassionate and dedicated Wound Treatment Nurse to join the Paradigm Healthcare family!Job DutiesProvide direct nursing care in accordance with the nurse practice act, federal and state regulations, facility policies and procedures, and prudent nursing judgmentPlan, develop, organize, implement, evaluate, and direct the delivery of a resident’s plan of care in accordance with current physician’s orders, rules, regulations, and guidelines that govern the nursing care facilitiesHave knowledge of and utilize of the facility’s written policies and procedures that govern the day-to-day functions of the nursing service departmentUtilize established methods for coordination and communication of nursing services with other resident services to ensure the continuity of the residents’ total regimen of careParticipate in facility surveys (inspections) made by authorized government agencies as necessary or as may be directedConduct themselves in a professional manner in compliance with facility policiesAgree not to disclose assigned user ID code and password for accessing resident/facility information and promptly report suspected or known violations of such disclosure to the AdministratorAgree not to disclose resident’s protected health information and promptly report suspected or known violations of such disclosure to the AdministratorReport any known or suspected unauthorized attempt to access facility’s information system.Participate in developing, evaluating, revising, and implementing a plan of care for each resident to achieve and maintain the highest practicable level of wellbeing possibleProvide education to residents/families/caregivers when the need arises, or as directed by Administration and/or physicianProvides prompt emergency support measures, including CPR, in accordance with physician orders, resident/family wishes, advance directives, and facility policies and procedures.Provide care in a professional/courteous manner by ensuring the resident is approached in a kind, gentle and friendly mannerProvide care in a manner that respect for the resident's dignity and privacy is consistently upheldEnsure that nursing interventions are performed in a timely manner.
