Job summary

Paradigm at Faith Memorial is seeking a compassionate Wound Treatment Nurse to join our growing team in a skilled nursing facility. You will provide direct nursing care, develop and direct care plans, and ensure adherence to regulatory guidelines.

CPR and emergency support are required, with a focus on resident dignity and timely interventions. The ideal candidate holds an active RN or LVN license in Texas, an ADN, and 2 years in long-term care with comfort in restorative practices and

Qualifications

  • Must possess a current, unencumbered RN or LVN license in the state where practice is required.
  • An Associate Degree in Nursing is required.
  • Two years’ experience in Long-Term Care preferred.
  • Six months experience in rehabilitative and restorative nursing practices preferred.
  • Strong knowledge of Point Click Care preferred.
  • Computer literacy skills (Microsoft Outlook, Microsoft Office, Internet).

Responsibilities

  • Provide direct nursing care in accordance with the nurse practice act, federal and state regulations, facility policies, and prudent nursing judgment.
  • Plan, develop, organize, implement, evaluate, and direct the delivery of a resident’s plan of care.
  • Coordinate nursing services with other resident services to ensure continuity of care.
  • Participate in facility surveys and inspections as required.
  • Provide CPR and emergency support as directed by physician orders and policies.
  • Develop daily work and unit assignments; conduct rounds to ensure standards are met.
  • Educate residents, families, and caregivers as directed by Administration.
  • Report hazards, incidents, or unauthorized access to facility information systems per policy.

Calling all candidates who want to be part of a fun and growing team!We are a skilled nursing facility seeking an experienced, compassionate and dedicated Wound Treatment Nurse to join the Paradigm Healthcare family!Job DutiesProvide direct nursing care in accordance with the nurse practice act, federal and state regulations, facility policies and procedures, and prudent nursing judgmentPlan, develop, organize, implement, evaluate, and direct the delivery of a resident’s plan of care in accordance with current physician’s orders, rules, regulations, and guidelines that govern the nursing care facilitiesHave knowledge of and utilize of the facility’s written policies and procedures that govern the day-to-day functions of the nursing service departmentUtilize established methods for coordination and communication of nursing services with other resident services to ensure the continuity of the residents’ total regimen of careParticipate in facility surveys (inspections) made by authorized government agencies as necessary or as may be directedConduct themselves in a professional manner in compliance with facility policiesAgree not to disclose assigned user ID code and password for accessing resident/facility information and promptly report suspected or known violations of such disclosure to the AdministratorAgree not to disclose resident’s protected health information and promptly report suspected or known violations of such disclosure to the AdministratorReport any known or suspected unauthorized attempt to access facility’s information system.Participate in developing, evaluating, revising, and implementing a plan of care for each resident to achieve and maintain the highest practicable level of wellbeing possibleProvide education to residents/families/caregivers when the need arises, or as directed by Administration and/or physicianProvides prompt emergency support measures, including CPR, in accordance with physician orders, resident/family wishes, advance directives, and facility policies and procedures.Provide care in a professional/courteous manner by ensuring the resident is approached in a kind, gentle and friendly mannerProvide care in a manner that respect for the resident's dignity and privacy is consistently upheldEnsure that nursing interventions are performed in a timely manner.
