Job summary

Paradigm Healthcare is seeking an experienced Wound Treatment Nurse to join our skilled nursing facility. You will provide direct nursing care, plan and direct resident care, and coordinate with teams to ensure high-quality outcomes while respecting resident dignity and privacy.

Responsibilities include implementing physician orders, CPR when needed, and educating residents and families. We value compassionate, professional care and compliance with all regulations and policies.

Qualifications

• Must possess a current, unencumbered license to practice as an RN or LVN in Texas.
• Associate Degree in Nursing required.
• Two years of Long-Term Care experience preferred.
• Strong communication and documentation skills.
• Proficiency with Point Click Care or similar care management software preferred.

Responsibilities

• Provide direct nursing care per nurse practice act and facility policies.
• Develop and direct the plan of care based on physician orders and regulations.
• Coordinate with other resident services to ensure continuity of care.
• Provide education to residents and families as needed.
• Assist with CPR and emergency responses as required.
• Document and report incidents, exposures, and safeguarding information.
