Job summary

CareMed Health Solutions is seeking a part-time Wound Care Nurse Practitioner to provide wound services to residents in long-term care facilities. You will diagnose and treat wounds, prescribe medications (including controlled substances per KY law), order and follow-up with treatments and tests, and communicate changes with nursing home staff to update the care plan.

The ideal candidate has a current, unrestricted KY NP license, board certification, experience with chronic and vascular wounds,

Qualifications

Current, unrestricted NP license in Kentucky.
Board Certification as NP.
Clinical background in adult, family or geriatric specialties.
Clinical background of wound care including chronic, debilitating, stalled or infected wounds.
Comfort diagnosing and treating diabetic, arterial, venous and mixed etiology wounds; Kennedy ulcers okay.
Familiar with staging pressure injuries per NPIAP; wound care products; wound vacs and DME.

Responsibilities

Provide wound care services for long-term care patients in skilled nursing facilities.
Prescribe appropriate medications, including controlled substances per KY law.
Order, review and follow up with treatments and diagnostic testing.
Communicate with nursing home staff regularly about changes in condition and care plan.
Collaborate with the primary care physician to coordinate patient care.
Work within an interdisciplinary care team to optimize outcomes.
