Job summary

Mercyhealth in Freeport, IL is seeking a Nurse Practitioner, Wound Care, to provide compassionate, culturally sensitive care in a clinical setting while leading the wound care program and educating frontline staff.

The Wound Care NP will perform general and complex wound assessments, develop treatment plans, coordinate care with General Surgery and other clinicians, and document according to standards of care.

Qualifications

• Graduate of an accredited nursing program.
• Graduate of an accredited Master of Nursing Program.
• May require specific years of applicable experience to qualify per department guidelines.

Responsibilities

• Utilize the nursing process and collaborate with interdisciplinary team members in caring for patients.
• Maintain timely consult schedule.
• Coordinate care with General Surgery physician and other health professionals as needed.
• Assist in developing Wound Care Program protocol and processes.
• Administer prescribed medications and treatments per approved nursing techniques, policies and procedures, standards of care, and State laws.
• Document according to standards of care.
• Initiate a patient plan of care according to individualized needs of the patient as prescribed by Physician and hospital policy.
• Participate in department or unit performance improvement activities.
• Assist in the development of a Systemwide wound care team.
• Assure wound care practices remain current using evidence-based care.
• Document complete wound care assessments and treatments.
• Initiate a wound care patient plan of care including patient and family instruction.
