Job summary

CareMed Health Solutions is seeking a Wound Care Nurse Practitioner to provide wound services to residents in long-term care facilities. The role includes prescribing medications per Kentucky law, ordering treatments, and coordinating with the interdisciplinary team.

Ideal candidates hold a Kentucky NP license, board certification, and have wound care experience in a long-term care setting. A DEA license is optional; a sign-on bonus is offered.

Qualifications

• Current, unrestricted NP license in Kentucky.
• Board Certification.
• DEA licensure (Optional).
• Clinical background in adult, family, or geriatric specialties.
• Clinical background of wound care (chronic, debilitating, stalled, or infected wounds).
• Ability to diagnose and treat diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers.
• Ability to stage pressure injuries per NPIAP.
• Familiar with wound care products (collagens, alginates, foams, silvers).
• Experience with wound vacs and DME.
• Wound certification preferred or willing to obtain.
• Experience in long-term care setting.
• Ability to work with an EMR.
• Ability to coordinate patient care within an interdisciplinary team.

Responsibilities

• Provide wound care services for long-term care patients in skilled nursing facilities.
• Prescribe medications, including controlled substances per KY law.
• Order, review, and follow up treatments and diagnostic testing.
• Communicate with nursing home staff about changes in condition and care plan.
• Collaborate with the primary care physician.
