Job summary

CareMed Health Solutions in Kentucky is seeking a Primary and Wound Care Nurse Practitioner to deliver wound care and primary services for residents in long-term care facilities. You will diagnose, treat, prescribe medications, and coordinate with facility staff to adjust care plans.

The role requires a KY NP license, board certification, and experience with chronic wounds, wound care products, wound vacs, and EMR systems. DEA licensure is optional.

Qualifications

• Current, unrestricted NP license in Kentucky.
• Board Certification required.
• DEA licensure optional.
• Clinical background in adult, family or geriatric care.
• Clinical wound care background including chronic or infected wounds.
• Comfort diagnosing and treating vascular/diabetic wounds and Kennedy Ulcers.
• Ability to stage pressure injuries per NPIAP.
• Familiar with wound care products (collagens, alginates, foams, silvers).
• Experience with wound vacs and related DME.
• Wound certification preferred or willingness to obtain.
• Experience in long-term care; EMR proficiency.
• Ability to work in an interdisciplinary care team; basic coding knowledge preferred.

Responsibilities

• Provide wound care and primary services for long-term care residents in skilled facilities.
• Prescribe medications per Kentucky law; include controlled substances as allowed.
• Order and review treatments and diagnostics; follow up as needed.
• Communicate with nursing home staff about changes in condition and care plans.
• Collaborate with primary care physicians on patient care plans.
