Job summary

CareMed is seeking a Wound NP to provide comprehensive wound care for residents in long-term care facilities. You will diagnose, treat, and manage chronic wounds, coordinate with facility staff, and collaborate with primary care physicians to optimize outcomes.

This role requires Kentucky NP license, board certification, and experience with wound care and wound vacs; DEA license optional. You will work as part of an interdisciplinary team and use EMR systems.

Qualifications

Current, unrestricted NP license in Kentucky.
Board Certification.
DEA licensure (Optional).
Clinical background in adult, family, or geriatric specialties.
Clinical background of wound care; including chronic, debilitating, stalled, or infected wounds.
Diagnose and treat diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers.
Staging pressure injuries according to the NPIAP.
Familiar with wound care products; collagens, alginates, foams, silvers, etc.
Experience with wound vacs and DME.
Wound certification preferred or openness to getting certified.
Experience in long-term care setting.
EMR proficiency.
Interdisciplinary care team coordination.

Responsibilities

Provide wound care services for long-term care patients in skilled nursing facilities.
Prescribe medications, including controlled substances per KY law.
Order, review, and follow up with treatments and diagnostic testing.
Communicate with nursing home staff about changes in condition.
Coordinate with the care plan and interdisciplinary team.
Collaborate with the primary care physician.
