Job summary

Zohorecruit seeks an experienced Director of Wound Care (Nurse Practitioner) to lead our wound care service line across partnered skilled nursing, assisted living, and long-term care facilities. The role blends hands-on clinical work with strategic leadership and requires regular travel to partner facilities.

The incumbent will oversee advanced wound procedures, mentor clinicians, own performance metrics, ensure compliant documentation and billing, and drive service growth while improving

Qualifications

• Active NP license with multi-state preference.
• Board certification in wound care (WCC/CWCN/CWS or equivalent).
• Minimum 5 years of hands-on wound care experience.
• Minimum 2–3 years in clinical leadership or program management.
• Experience in skilled nursing/long-term care settings.
• Familiarity with CMS documentation and billing for wound care procedures.

Responsibilities

• Lead the wound care service line across partner facilities, focused on clinical oversight and growth.
• Maintain clinical competency in advanced wound procedures and mentor staff.
• Oversee clinicians in debridement, skin grafting, and related procedures.
• Establish and maintain evidence-based wound assessment, treatment planning, and escalation protocols.
• Ensure appropriate diagnostic workups and documentation, including labs and imaging.
• Support clinicians in identifying wound etiologies and healing barriers.
• Monitor and improve clinical outcomes, documentation quality, and financial performance.
• Own and review the KPI dashboard for the wound care service line.
• Ensure accurate documentation, coding, and billing for procedures.
• Collaborate with partner facilities to reduce avoidable hospitalizations and ED visits.
• Foster relationships with SNFs, AL, and LTC partners; oversee team development and certification compliance.
• Travel regularly to partner facilities for hands-on oversight and mentorship.
