Job summary

Cypress Valley Center for Nursing and Rehabilitation, part of the Alliance Health Group, is seeking a Wound Care / Admissions Nurse (RN or LPN) to join our clinical team. This role blends wound care with admission duties to impact residents from day one, delivering hands-on care and coordinated care planning.

Applicants should hold an active RN or LPN license, have clinical nursing experience, and CPR certification.

Qualifications

• Current RN or LPN license in the state of practice.
• Previous clinical nursing experience required.
• Wound care experience strongly preferred.
• Long-term care or skilled nursing experience preferred.
• Experience with admissions is a plus.
• Current CPR certification.
• Strong clinical assessment and documentation skills.
• Knowledge of wound prevention, treatment, and infection control practices.
• Excellent communication and organizational skills.
• Ability to prioritize multiple clinical responsibilities.
• Compassionate, dependable, and resident-focused approach.
• Ability to work independently while collaborating with an interdisciplinary team.

Responsibilities

• Wound care: perform wound and skin assessments on admission and during stay.
• Develop and monitor individualized wound care plans per orders and protocols.
• Provide wound treatments and preventive skin care.
• Conduct rounds to monitor wound progression and skin integrity.
• Communicate changes to the care team and providers.
• Collaborate with nursing, dietary, therapy, and physicians to support healing.
• Monitor nutrition/hydration related to wound healing.
• Maintain accurate wound documentation and records.
• Keep wound care supplies organized and properly stocked.
• Adhere to infection prevention and safety standards.
• Admissions: assist with admission process for new residents.
• Complete nursing assessments and obtain accurate clinical information on admission.
• Review hospital records to identify diagnoses, meds, wounds, and needs.
• Assess skin integrity and identify existing or potential wounds on admission.
• Communicate admission findings to the team and ensure orders/treatments are implemented.
• Provide education to residents and families during admission.
