Eastern Health Group is seeking a dedicated Wound Care Nurse (RN or LPN) to assess and manage wounds, ostomies, and skin conditions in residents. You will develop care plans and collaborate with a multidisciplinary team to meet clinical standards.

Qualifications

• Active Virginia RN or LPN license is required.
• Wound care certification preferred.
• 1–2 years of clinical wound care experience required, preferably in long-term care.
• Strong knowledge of wound care products, protocols, and documentation.

Responsibilities

• Conduct regular skin assessments on residents to identify wounds, pressure injuries, or areas at risk.
• Develop and implement individualized wound care plans in collaboration with the interdisciplinary team.
• Perform wound treatments, dressing changes, and other prescribed interventions.
• Monitor wound healing and document progress accurately and consistently.
• Educate staff on wound prevention strategies and proper dressing techniques.
• Collaborate with physicians, dietitians, therapists, and other health professionals for comprehensive care.
• Maintain regulatory compliance related to wound care in long-term care settings.
• Participate in QAPI programs.
