Job summary

Waverly Rehabilitation And Healthcare Center is hiring a Wound Care Nurse (F/T) to develop and implement individualized wound care plans in collaboration with an interdisciplinary team. The role emphasizes skin preservation, protocol approval by medical staff, and hands-on care for challenging wounds.

Responsibilities include participating in multidisciplinary rounds, organizing restorative programs, and ensuring regulatory compliance.

Qualifications

• RN or LPN license current in Virginia.
• LTC/SNF experience preferred.

Responsibilities

• Develop and ensure the wound care plan for each patient with the interdisciplinary team.
• Preserve skin integrity by implementing skin preservation strategies.
• Ensure wound and skin care protocols are approved by medical staff.
• Participate in weekly multidisciplinary wound and skin rounds for high-risk patients and those with ulcers.
• Provide hands-on wound care for difficult/non-healing wounds.
• Organize and implement the restorative program.
• Ensure all regulatory requirements are met.

Summary/Objective:

In this role, you will develop and ensure the wound care plan specific for each patient in collaboration with an interdisciplinary team.

• Preserving the integrity of the skin by implementing and monitoring skin preservation strategies.
• Ensuring all wound and skin care protocols are approved by the medical staff.
• Participatingmultidisciplinarywound and skin rounds on a weekly basis for high risk patients and patients with current pressure ulcers and other significant wounds.
• Providing hands‑on wound care for all difficult and non‑healing wounds.
• Organize and implement the restorative program.
• Ensuring all regulatory requirements are met.

Qualifications:

• Current State License as an RN/LPN
• LTC/SNF experience preferred
