Job Description

We are seeking self‑motivated, energetic, detail‑oriented, highly organized, tech‑savvy Registered Nurses to join our Workers’ Compensation Field Case Management team. This is a work‑from‑home position with travel to meet clients, providers and employers primarily around Monroe, MI & Toledo, OH, up to approximately 75 miles. The case manager will coordinate and facilitate medical treatment of workers’ comp clients, document all activities and support return‑to‑work processes.

Responsibilities

• Coordinate and facilitate medical treatment of workers’ compensation clients in various stages of recovery.
• Travel to meet clients, providers and employers; maintain regular communication to facilitate return to work.
• Conduct assessments, identify medical and vocational barriers, and develop strategies to overcome barriers.
• Attend appointments with clients and providers to review medical status and discuss treatment plans.
• Compare recommended treatment plans with clinical protocols and collaborate with providers to resolve differences.
• Maintain detailed documentation, including case analysis of job requirements, and provide summary reports within 24 hours to referral sources.
• Complete catastrophic referral training within the first year and manage occasional catastrophic referrals after regular business hours.
• Use technology (iPhone, laptop, Microsoft Office) for timely, concise communication and record‑keeping.

Qualifications/Experience

• 3+ years clinical practice experience.
• 3+ years case management experience with workers’ compensation, disability, and auto liability preferred.
• If no case management experience, 5 years of Home Health Care and/or Hospice care.
• Bilingual in Spanish preferred.
• Knowledge of laws and regulations governing delivery of rehabilitation services.
• Effective communication, organizational, and interpersonal skills.
• Ability to work independently from a safe and secure home environment.
• Proficiency with Microsoft Office and corporate software applications.

Education

Bachelor’s degree or equivalent experience.

Licenses & Certifications

Registered Nurse (RN) required. Preferred credentials: CCM, CDMS, COHN, COHN‑S, CRRN.
