Job summary

Phoenix Rescue Mission, a faith-based nonprofit in Phoenix, AZ, seeks a dedicated case manager to support men, women, and children facing hunger, homelessness, addiction, and trauma. You will assess clients, create personalized plans, and coordinate essential services with partners to promote stability and recovery.

Responsibilities include crisis response, benefits assistance, documentation, and strong collaboration with multidisciplinary teams.

Qualifications

• Bachelor's degree in Social Work, Psychology, Counseling, Human Services, or related field preferred, or at least four years of experience in case management or client services.
• Experience with homelessness, substance use disorders, mental illness, or co-occurring disorders preferred.
• Experience using electronic case management systems and regulatory documentation preferred.
• Fluency in Spanish preferred.
• Strong organizational, communication, and relationship-building skills.
• Proficiency with Microsoft Office and case management documentation systems.
• Ability to exercise sound judgment and respond effectively in crisis situations.
• Solid Christian witness and lifestyle.

Responsibilities

• Conduct comprehensive client assessments and develop individualized service plans.
• Maintain regular contact with clients and monitor progress toward goals.
• Coordinate support services and connect clients with medical, behavioral health, housing, and employment resources.
• Assist clients with benefits applications (AHCCCS, SNAP, childcare).
• Develop and maintain relationships with community partners and outside agencies.
• Maintain accurate case documentation in compliance with policies.
• Schedule appointments, referrals, transportation, and follow-ups.
• Participate in case conferences and multidisciplinary team meetings for coordinated care.
• Respond to client crises using trauma-informed practices and de-escalation.
• Transport clients to appointments as needed.
• Support clients in self-administration of medications per physician orders.
• Lead or participate in devotions and organizational activities reflecting a Christian witness.
