Bi-Bett Corporation in Walnut Creek, CA, seeks a Women’s and Children’s Case Manager for its residential substance use disorder program. You will provide on-site case management for women and their children, addressing housing, healthcare, childcare, transportation, employment, education, and benefits to support recovery.

Join a multidisciplinary team delivering trauma-informed, person-centered services while maintaining client confidentiality and professional boundaries in a residential setting.

Qualificações

• High school diploma or equivalent required.
• Experience in case management, social services, behavioral health, or SUD treatment.
• Knowledge of Medi-Cal, CalFresh, WIC or other public benefits preferred.

Responsabilidades

• Provide consistent, on-site case management services to residents throughout their treatment episode.
• Conduct needs assessments and establish individualized goals with clients.
• Assist clients with applications, forms, and accessing benefits and services.
• Navigate healthcare, housing, childcare, transportation, and employment resources.
• Coordinate referrals with healthcare providers, county programs, schools, and social services.
