Qualifications

Responsibilities

• Provide direct patient care within an area of clinical specialization.
• Educate patients, families, and caregivers using individualized teaching and counseling.
• Participate in assessment, implementation, and ongoing evaluation of clinical services for a designated patient population.
• Conduct patient screening, family education, discharge planning, and follow-up.
• Engage in case management and research activities.
• Maintain clinical expertise in the area of specialization.
• Serve as a resource for patients and families/caregivers.
• Promote interdisciplinary approach to patient care delivery.
• Act as a patient advocate utilizing the Patient Bill of Rights.
