Job summary

Pathways Healthcare in Massachusetts is seeking a Wound/Ostomy Nurse for our Home Health program. You will provide expert wound and ostomy care in patients' homes, developing and revising plans of care in collaboration with physicians and the care team.

We value bedside manner, strong communication, and a commitment to patient-centered care. The role is full-time, with a Monday–Friday schedule in the Boston area, serving Boston and Southshore communities from our Westwood office.

Qualifications

• Must hold an active Nursing license in MA.
• Must hold an active WOCN certification.
• Preferably 2+ years of nursing experience, geriatrics focus.
• Nursing degree from an accredited institution.
• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver''s license and own a reliable, insured vehicle.

Responsibilities

• Initiate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather medical histories.
• Develop and revise individualized plans of care based on patient status.
• Collaborate with physicians, nurse practitioners, and interdisciplinary teams to create Plans of Care.
• Address inquiries from patients, caregivers, and medical teams.
