Job summary

Fora Health Inc in Oregon is seeking a Withdrawal Management Nurse to provide comprehensive nursing care for individuals undergoing withdrawal from substances. The role involves assessing patients, developing treatment plans with a multidisciplinary team, and monitoring responses during withdrawal.

You will administer medications, document progress, educate patients on withdrawal and relapse prevention, and coordinate with nursing assistants.

Qualifications

Must be a licensed RN in Oregon or able to obtain licensure
BLS certification required
Experience with patient care in a fast-paced environment preferred

Responsibilities

Assess patients' physical and mental health, including withdrawal history and vital signs
Develop individualized treatment plans with a multidisciplinary team
Administer medications and monitor patient responses
Document progress and maintain accurate medical records
Educate patients on withdrawal, treatment options, and relapse prevention
Collaborate with nursing assistants and other staff to ensure protocol adherence
Respond to medical emergencies and stabilize patients as needed
