Job summary

UnitedHealth Group is seeking Advanced Practice Clinicians (NP/PA) to conduct in-home health assessments across Wisconsin. The statewide traveler role involves 85% travel with travel expenses covered, a base salary plus incentives, and flexible scheduling within a 7-day week.

This non-prescribing, field-based role focuses on education, gap closure, and collaboration with PCPs to improve health outcomes for Medicare Advantage and other plan members.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geria/Adult-Gerontology Primary Care.
• Current NCCPA certification for PAs and state licensure.
• Active, unrestricted licensure where you practice.
• At least 1 year of clinical experience, preferably home health.
• Driver's license and access to reliable transportation.

Responsibilities

• Past medical history review, vitals, and physical exam.
• Evidence-based screenings and point-of-care testing.
• Identify and document diagnoses for care planning.
• Communicate findings to members' PCPs to close gaps in care.
• Recognize urgent situations and intervene appropriately.
• Educate members on disease processes and medications.
• Address social determinants of health and provide referrals.
