Job summary

UnitedHealth Group's Optum Home & Community Care is hiring for a Home Health Nurse Practitioner or Physician Assistant to perform in-home assessments across Wisconsin. The role offers flexible scheduling, travel covered, and no on-call, weekends, or holidays.

You will identify diagnoses, coordinate care with PCPs, and connect patients with social services while delivering compassionate, patient-centered care.

Qualifications

• Active ANCC/AANP or NCCPA certification and state licensure
• 1+ years clinical experience (family, geriatric, or home health preferred)
• Active and unrestricted driver's license with reliable transportation for home visits

Responsibilities

• Conduct comprehensive in-home assessments (history, vitals, exam)
• Identify and document diagnoses for care management
• Communicate findings to members' PCPs to address gaps in care
• Recognize urgent/emergent situations and intervene appropriately
• Educate members on disease processes, medications, and adherence
• Address social determinants of health and provide referrals as needed
