Job summary

Health Imperatives seeks a WIC Family Support Coordinator for Cape Cod WIC. This part-time role (20 hrs/week) focuses on case management for high‑risk WIC participants, offering counseling, referrals, intake, and follow-up to ensure access to social services beyond nutrition.

The position requires a relevant bachelor’s degree and at least two years of counseling or case management experience, with strong outreach and collaboration across agencies. A Massachusetts driver's license is required.

Qualifications

• Bachelor’s degree in a relevant field; experience may substitute for up to 2 years.
• Minimum 2 years counseling or case management experience.
• Experience in social services such as housing, DV, substance use.
• Ability to work with diverse communities and provide outreach.

Responsibilities

• Provide counseling and referrals for support services.
• Complete intake and assessment for potential clients.
• Ensure follow-up to secure service receipt; coordinate with program staff.
• Develop triage for addressing non-nutritional needs.
• Maintain confidentiality policies and coordinate community referrals.
