Job summary

Qualifications

• Master's degree in a related field or Bachelor's with four years of experience in social services.
• Certification for SUD CM preferred and registration required; TB test within 45 working days; CPR/First Aid at hire or within 90 days.

Responsibilities

• Provide case management with crisis intervention, discharge planning, group & individual work, mental health/substance use counseling, screening, and assist clients with basic needs and symptom management.
• Complete accurate documentation and meet minimum billable requirements for direct client services when required.
• Facilitate placement including pre-placement visits and transportation.
• Shift coverage including part of the on-call team.
• Provide field-based services as required by program and client need.
• Maintain confidentiality of client care and participate in program meetings.
• Collaborate with ACT team and Program Manager to support transitions between ACT and ICM levels of care.
