Responsibilities

• Collects and analyzes patient data to develop and implement care plans.
• Directs care and coordinates with team and assistants.
• Collaborates with patient, family, physicians and staff to document outcome-focused plans.
• Participates in governance, committees, and unit projects.
• Documents responses to interventions and updates care plans accordingly.
• Uses evidence-based practice to guide care decisions.
