University of Kansas Medical Center is seeking a Community Health Worker for the West Kansas Region in Family Medicine. The CHW will engage communities, connect individuals to health and social resources, and support Thrive program initiatives across Finney, Ford, Grant, and Seward counties.

The role requires travel within the service area, bilingual Spanish abilities, and a commitment to building community capacity alongside primary care teams.

Qualifikationen

• High school diploma or GED required.
• Certified in KS as CHW; willing to take Core Curriculum within 6 months.
• Complete ECHOS Database training within 3 months of employment.
• Experience working with diverse populations in community settings.

Aufgaben

• Engage community-based organizations to build referral networks to address clients’ SDOH needs.
• Participate in clinic site meetings to plan strategies for Thrive engagement and equity goals.
• Attend CHW team meetings to discuss Thrive strategies and action items.
• Maintain ECHOS database with notes, forms and time tracking; ensure data quality and accessibility.
