Job summary

University of Kansas Medical Center’s Family Medicine department seeks a Community Health Worker to support the Central Kansas Region. This hybrid, full-time role covers counties including Reno, Rice, McPherson, Harvey, Stafford, Pratt, and Kingman, with regular travel to connect residents to health and social resources and Thrive program navigation.

The position requires residency in the service area or relocation, KS CHW certification (or plan to complete within 6 months), and bilingual

Qualifications

• Education: High school diploma or GED.
• Certifications: KS CHW certification or willingness to complete Core Curriculum within 6 months.
• Complete ECHOS Database internal training within 3 months of employment.
• Experience working with diverse populations in community settings, including those with mental health concerns.

Responsibilities

• Engage community-based organizations to build referral networks addressing clients' SDOH needs.
• Attend bi-monthly virtual learning collaboratives; receive education on community resources and best practices.
• Coordinate and communicate with CHW project leadership; attend meetings to discuss Thrive strategies.
• Complete administrative tasks; maintain organized records with staff.
• Maintain an up-to-date inventory of community resources and health initiatives.
