Job summary

Galen Medical Group is seeking an Annual Wellness Navigator I, MA (Non-Certified) to schedule procedures, preoperative requirements, and AWV-related tasks. You will coordinate appointments across clinic sites, pre-populate EMR records, and track preventive care measures for Medicare populations.

Responsibilities include contacting patients before AWVs, conducting health interviews via phone or telemedicine, and ensuring documentation completeness while adhering to HIPAA and CLIA standards.

Qualifications

• Customer service experience is required.
• Ability to work independently with attention to detail.
• Strong verbal and written communication skills.
• Proficient with PC-based systems and standard software.
• Knowledge of medical terminology is a plus.

Responsibilities

• Schedule patients for procedures and AWVs and advise on preop requirements.
• Coordinate appointments across clinic, ambulatory surgery center, or hospital.
• Pre-populate EMR with AWV data and track preventive care measures.
• Contact patients to define AWV goals and steps.
• Maintain reports on patient contact status and visit gaps.
