The Service Coordinator role at COAAA in Columbus, OH connects individuals to supportive services, conducts non-clinical assessments, and advocates for their needs to promote stability, wellness, and quality of life.

Responsibilities include completing wellness assessments, linking clients to nutrition programs, transportation, and meals, maintaining local resource directories, collaborating with providers, advocating for rights, and documenting case records in compliance with program rules.

Competenze

• High school diploma or equivalent required.
• 1 year of experience in social services, case management, or healthcare coordination.
• Valid driver’s license and reliable transportation with proof of insurance.
• Bachelor’s degree and CSA/PSC certification preferred.
• Experience with older adults or vulnerable populations encouraged.

Mansioni

• Complete and document non-clinical wellness and social assessments.
• Connect individuals to community resources such as SNAP, transportation, and meals.
• Maintain an up-to-date local resource directory.
• Collaborate with service providers, health providers, community organizations and partners.
• Advocate for clients’ rights and ensure person-centered care.
• Provide education on emergency preparedness and assist with planning.
• Ensure timely follow-up on referrals and maintain case records in compliance with program requirements.
