Job summary

Continuum Therapy Partners is seeking a Part-Time Wellness Coordinator to split between Asbury Heights in Pittsburgh, PA and Seneca Place in Verona, PA. You will develop and oversee wellness programs that boost residents' physical, emotional, and social well-being, working closely with rehab teams and caregivers to support daily living.

The role requires a hands-on, compassionate communicator who can track progress, ensure safety standards, and foster participation in events.

Qualifications

• High school diploma required.
• Bachelor’s degree in therapy or wellness related field preferred.
• 3–5 years experience in related field.
• Ability to plan and implement wellness programs and events.

Responsibilities

• Develop, oversee, and implement wellness programs to enhance residents’ physical, emotional, and social well-being.
• Collaborate with rehab department, caregivers, and residents to promote healthy living and improved quality of life.
• Track progress and adjust programs to align with facility goals and regulations.
• Ensure wellness activities comply with health and safety regulations and policies.
• Foster a culture of wellness and encourage participation in programs.
• Provide one-on-one support, coaching, or referrals as needed.
• Maintain clear communication with residents, therapy staff, and facility staff.
• Coordinate with physicians to obtain necessary orders or signatures.
• Assess wellness initiatives through feedback and data to improve outcomes.
• Perform administrative duties to support departmental operations.
