Continuum Therapy Partners is excited to offer a Part‑Time opportunity for a Wellness Coordinator split between Asbury Heights in Pittsburgh, PA and Seneca Place in Verona, PA—a supportive collaborative skilled nursing facility where your expertise makes a daily impact.

Essential Functions
- Develop, oversee, and implement wellness programs designed to enhance the physical, emotional, and social well‑being of residents.
- Work closely with the rehab department, caregivers, and residents to create a supportive environment that promotes healthy living and an improved quality of life.
- Track resident progress, adjust programs as needed, and ensure that all wellness initiatives are aligned with the facility’s goals and regulations.
- Ensure that all wellness programs and activities comply with relevant health and safety regulations and organizational policies.
- Foster a culture of wellness by motivating and encouraging participation in wellness programs.
- Offer one‑on‑one support, coaching, or referrals to additional resources as needed.
- Maintain clear and effective communication with residents, therapy staff, and facility staff.
- Work with DOR to ensure AAAASF needs are met.
- Routine checks/documentation of therapy equipment (freezers, hydrocollators, etc.).
- Gathering and organization of Outpatient Therapy documentation.
- Driving patients to the wellness center for treatment, as needed.
- Coordinate with Physicians to obtain orders or other signatures, as necessary.
- Assess the impact of wellness initiatives through feedback and data, and make necessary adjustments to improve outcomes.
- Perform administrative services to help departmental operations as assigned by supervisor.
- Adhere to all company rules and regulations.

Education, Experience, and Licensing Requirements
- High School Diploma required.
- Bachelor’s degree in a therapy or wellness related field preferred.
- 3‑5 years experience in related field.
