Job summary

BJC Corporate Health Services is recruiting a registered nurse for outpatient care coordination and direct patient care in a health‑focused corporate setting. You will perform assessments, develop care plans, and collaborate across a multidisciplinary team to deliver high‑quality nursing care.

Preferred candidates have a Nursing Diploma/Associate's degree with RN licensure; Bachelor's degree preferred and 2–5 years of experience.

• Education includes Nursing Diploma or Associate's degree; Bachelor's preferred.

Responsibilities

• Assess patient preferences, barriers, values, and cultural needs.
• Develop, implement and document care plans with defined goals.
• Collaborate with interprofessional team and family to communicate the plan of care.
• Promote equity and empathy in care delivery to diverse populations.
• Evaluate patient changes and communicate updates with the care team and family.
• Support ongoing advancement of nursing practice and participate in quality improvement.

• 2 shifts every 6 weeks
• 1 Saturday shift ever 12 weeks

Events can vary on time but typically see staff working from 8 am to 11 am.

Working with a variety of populations to conduct health/wellness assessments

Role Purpose

Provides direct patient care activities including assessment, diagnosis, planning, implementation, and evaluation within the guidelines of the standards of nursing care to patients who require a high level of clinical expertise.

Responsibilities

• Assess patient preferences and barriers to involvement in care, including their values, emotional, spiritual, cultural, and population-specific needs.
• Develops, implements, and documents individual plans of care with defined goals in collaboration with other members of the interprofessional team and patient, family or caregiver in accordance with the established guidelines and standards of nursing care. Proactively plans and ensures communication of the plan of care across the continuum of care.
• Promotes respect, equity and empathy in interactions with diverse and vulnerable populations through care delivery (e.g. support for emotional, spiritual, and cultural preferences of patient, family and/or caregivers). Practices collaborative problem solving, service recovery and advocacy for patient family centered continuity of care. Implements care by integrating data from the interprofessional team and critical thinking in a safe and timely manner.
• Demonstrates their individual role to the ongoing advancement of nursing clinical practice (e.g. participation in community health events, professional organization membership, quality improvement, continuing education activities).
• Evaluates changes in patient's condition, informs and collaborates with family and/or caregivers, and communicates with interprofessional team as changes occur in plan of care, updates plan of care in EMR. Evaluates current nursing care to ensure evidence-based practice and quality patient outcomes.
• BJC has determined this is a safety-sensitive position. The ability to work in a constant state of alertness and in a safe manner is an essential function of this job.
