Qualifications

• Valid RN or LPN license.
• Effective communication with residents, families, and other healthcare professionals.

Responsibilities

• Collaborate with a team to develop, implement, and review personalized care plans for residents.
• Conduct health assessments and identify medical concerns or changes; coordinate with other professionals.
• Administer medications as prescribed and monitor effectiveness and side effects.
• Maintain accurate medical records, care plans, and incidents for regulatory compliance.
• Respond promptly to medical emergencies and coordinate with emergency services if needed.
• Communicate openly with residents' families about health changes and care decisions.
• Advocate for residents' rights and preferences in all aspects of care.
