Job summary

Caring Hearts, Enriching Lives is seeking a Wellness Nurse (RN/LPN) for our assisted living community. This full-time position offers 32 hours per week, with a 12pm-8pm shift and alternating weekends, focusing on comprehensive resident care and accurate records.

As part of the wellness team, you will administer medications, assess residents, develop care plans with the team, and collaborate with healthcare professionals.

Qualifications

• Valid and active RN or LPN license.

Responsibilities

• Collaborate with team to develop, implement, and review personalized care plans for residents.
• Conduct health assessments and coordinate with other healthcare professionals to create effective care plans.
• Administer medications as prescribed, ensuring accuracy and monitoring effects.
• Maintain accurate medical information, care plans, and incident reports for compliance.
• Respond promptly to medical emergencies and coordinate with emergency services when needed.
• Communicate with residents' families about health status and changes.
• Advocate for residents' rights and preferences in all aspects of care.
