Job summary

GateWay at River City is seeking a dedicated Wellness Nurse (LPN) to join our Clinical Team. You will provide compassionate, high-quality nursing care while supporting residents' health, safety, and well-being.

This role offers meaningful relationships and the chance to make a positive difference every day. As an LPN, you’ll manage medication orders and treatments, coordinate admissions and discharges, and educate residents and families on care plans.

Qualifications

• A current LPN license in good standing.
• Two or more years working with seniors is helpful.
• Healthcare experience preferred in senior living.

Responsibilities

• Manage medication orders and documentation in compliance with regulations.
• Administer medications and treatments as ordered, with accurate documentation.
• Coordinate care by completing admissions, assessments, transfers, and discharges.
• Follow up with service providers to ensure resident needs are met.
• Educate and support residents and families on care plans and health needs.
• Provide friendly, professional customer service to residents and families.
• Support and oversee QMAs and CNAs, assisting with resident care as needed.
