Silver Birch Living, LLC. in Peoria, IL is seeking a dedicated Wellness Nurse (LPN) to join our clinical team. You’ll provide compassionate nursing care, manage medications, and support residents’ health and daily living.

You will build meaningful relationships with residents and families, collaborate with CNAs and QMAs, and contribute to a caring community where quality care is the priority. This is a rewarding role in an expanding organization focused on wellness and accessibility.

Qualifications

• High school diploma or GED.
• Healthcare experience, preferably in senior living.
• Two or more years working with seniors.
• Strong customer service skills and a compassionate approach to care.

Responsabilités

• Manage medication orders and documentation in compliance with regulations.
• Administer medications and treatments as ordered with accurate documentation.
• Coordinate care by completing admissions, assessments, transfers, and discharges.
• Follow up with service providers to ensure resident needs are met.
• Educate and support residents and families on care plans and health needs.
• Provide friendly, professional customer service to residents and families.
• Support and oversee QMAs and CNAs, assisting with resident care as needed.
