Job summary

The Neighborhood at Vestavia Hills, operated by Phoenix Senior Living, seeks a Wellness Nurse/Nurse Med Tech for a night shift from 7pm to 7am. The LPN reports to the Wellness Director and provides leadership in resident care and wellness services.

Requirements include LPN or RN licensure, at least one year of elder care experience (preferred), and flexibility to work weekends and evenings. The role emphasizes medication administration, care coordination, and quality assurance.

Qualifications

• Must be a Licensed Practical Nurse (LPN) or Registered Nurse (RN).
• Minimum of one year of experience working with elders or disabled individuals (preferred).
• Must have demonstrated leadership capabilities.
• Knowledge and experience in Assisted Living, Dementia care or Skilled Nursing (preferred).
• Ability to work weekends, evenings and flexible hours.

Responsibilities

• Monitor health and wellbeing of residents and provide leadership support with resident care and services.
• Lead, demonstrate and support all wellness elements of Phoenix programs and services.
• Medication care administration and documentation for residents.
• Communicate with resident support services and families.
• Maintain clinical quality assurance and monthly wellness visits.
