Job summary

The Neighborhood at Grove Park is seeking Nurses to join their team. The LPN reports directly to the Wellness Director and will work night shifts to support resident care.

The role emphasizes medication administration, wellness monitoring, and leadership in daily activities, with weekend/evening availability required to meet peak service times and ensure high-quality care for residents in our Alabama community.

Qualifications

• Must be a Licensed Practical Nurse or Registered Nurse.
• Minimum of one year of experience working with elders or disabled individuals.
• Demonstrated leadership capabilities.
• Experience in Assisted Living or Dementia care preferred.

Responsibilities

• Identify ongoing needs and services through the assessment / Personal Service Plan (PSP) process.
• Ensure proper follow through and documentation for changes in clinical care.
• Assist with monthly wellness visits and PSP to reflect resident monthly needs.
• Inform Wellness Director of changes in medication or service level.
• Assist with obtaining weights and vital signs monthly.
• Assist with direct resident care as needed.
