Wellness Nurse (LPN/RN)

30 to 40 Hours - Full Time Professional Bloomington, IL, US

Salary Range: $32.00 To $38.50 Hourly

Clinical

  • Performing nursing procedures within the scope of practice as allowed by state law andin compliance with Villas of HollyBrook.
  • Ensure sound infection control practices are in place that promotes ahealthyenvironment.
  • Contribute to resident assessments and customize service plan development.
  • Lead the medication administration process by correctly transcribing physicianorders,correctly ordering medication and completing error free medicationreminders.
  • Provide first aid and emergency assistance while on duty within thecommunity.
  • Participate in resident and family careconferences.
  • Maintain all privacy rules and regulations pertaining to the Health InsurancePortabilityand Accountability Act(HIPPA)
  • Ensure that changes in resident condition are evaluated, monitored, documented andthateffective interventions areimplemented.
  • Review resident records on a regular basis and ensure that documentationaccuratelyaddresses the healthcare issues, including identified concerns, interventionsandoutcomes, of allresidents.
  • Ensure that resident care is provided in a safe and effective manner, whileconsistentlymaintaining resident dignity, choice, privacy, andrespect.
  • Maintain a current knowledge of OSHA and state regulations and routinelymonitorCommunity compliance with regulations-related to resident care and nursingpractices.
  • Any other duties assigned by WellnessDirector.

Supervisory Functions

  • Assist with supervising Wellness Department employees in providing day-to-daycare.
  • Provide orientation and ongoing training asrequested.
  • Communicate and enforce policies and procedures including Villas of HollyBrookpolicies and procedures dealing with rules of employee conduct, the employer'ssafetyand sanitation policies and otherpolicies.

Clinical Nurse Functions

  • Scheduling podiatry visits, assist podiatry during the visit, if necessary, complete documentation in PCC
  • Incident reports: Complete following an incident, notify PCP, provide documentation and timeline to Wellness Director following company policy for incident follow-up
  • Scheduling
  • Medications: check in medications, add or remove any medications based on changes, fax new orders to pharmacy, deliver medications as needed to resident apartments and place in locked cabinet, pre-fill insulin syringes, remove medications after resident moves out, weekly narcotics count, administer medications if resident is unable to do it themselves
  • TB testing: administer 2-step TB for any employee or resident who needs it
  • Other duties as assigned by Wellness Director

Customer Service

  • Perform job duties for residents and with team members in a courteous and professional manner.
  • Take initiative to ensure resident safety and satisfaction is apriority.
  • Meetandgreetvisitorsinafriendly,helpfulmanner(Visitorsincludeanyonewhovisitsthecommunity;specifically,currentresidents'familiesandfriends,prospectiveresidentsand their families referral sources, vendors andregulators).
  • Answer phones appropriately, according to company and Communitystandards.
  • Communicate professionally and cooperate with Executive Director and allCommunity personnel.
  • Ensure compliance and understanding of all regulations regarding residents’ rights.

Training

  • Attend all required in-service and training

Other

  • Follow & communicate Villas of Holly Brook policies and procedures.

Position Requirement

  • Licensed RN/LPN in good standing with State Licensing Authority.
  • Experience of one year or more in the care of senior adults and adults withcognitivedecline ispreferred.
  • Ability to communicate effectively with residents, families, staff, community andState officials and generalpublic.
  • Must have compassion for and desire to work with theelderly.
  • Must demonstrate the ability to work responsibly as a team member as well asanindividual.
  • Must be honest, fair, dependable, respect confidentiality and the rights and privacyofothers.
  • Must practice and promote Villas of Holly Brook Policy and Procedures andMission Statement.
  • Must have exceptional organizational skills and ability to re-prioritize daily tasks inorderto accommodate fluctuating needs of the resident andCommunity.
  • Must have strong documentationskills.
  • Must pass criminal background check.
  • Must meet all health requirements.
  • Must be able to perform duties and responsibilities (essential job functions), with or without reasonable accommodations.

Physical and Mental Requirements

  • Medium work; ability to exe1i 20-50 pounds of force occasionally, and /or 10-25 pounds of force frequently, and /or greater than negligible up to 10 pounds of force constantly to lift, carry, push, pull or otherwise move objects.
  • Must be able to relate to and work with the ill, disabled, elderly, emotionally upset and at times hostile people within the community.
