Job summary

The Bungalows at Springdale is seeking a Wellness Nurse to join their team. The LPN (Nurse Med Tech) reports directly to the Wellness Director and works PRN days and weekends, with full-time options.

Responsibilities include medication administration, coordinating with resident support services and families, and maintaining clinical quality while leading wellness initiatives. Education: LPN or RN licensure, with at least one year of elder/disabled care experience and leadership skills;

Qualifications

• Must be a Licensed Practical Nurse (LPN) or Registered Nurse (RN).
• Minimum of one year of experience working with elders or disabled individuals, preferred.
• Must demonstrate leadership capabilities and flexible weekend/evening hours.

Responsibilities

• Provide resident care and monitor health per PSP process.
• Identify ongoing needs through assessment/PSP and ensure proper documentation.
• Assist Wellness Director with monthly wellness visits and PSP updates.
• Notify Wellness/Executive Director of changes in medication or service levels.
• Record weights and vital signs monthly and support wellness updates.
• Assist with direct resident care as needed.
