Job summary

The Bungalows at Fayetteville is seeking a Wellness Nurse/Nurse Med Tech to join their team. The LPN or RN reports to the Wellness Director and supports resident care and wellness initiatives.

Responsibilities include medication care administration, coordinating with families and services, and leading wellness activities while maintaining quality assurance. Weekend availability is required for peak service delivery.

Qualifications

• Must hold an LPN or RN license.
• Demonstrated leadership capabilities in care settings.
• Experience in assisted living, dementia care, or skilled nursing preferred.

Responsibilities

• Identifies ongoing needs and services of residents through the PSP process.
• Ensures proper follow-through and documentation for changes in clinical care needs.
• Assists Wellness Director with clinical sections of monthly wellness visits and PSP documentation.
• Notifies Wellness/Executive Director of changes in medications or services that may impact levels of care.
• Assists with obtaining weights and vitals monthly for each resident before wellness updates.
• Provides direct resident care as needed.
