Job summary

The Bungalows at Springdale is seeking a Wellness Nurse to join their team. The LPN (Nurse MedTech) reports to the Wellness Director and supports resident care and services with leadership. PRN days and every other weekend are required.

Responsibilities include medication care administration, PSP-based assessments, and maintaining clinical quality assurance while coordinating with families and support services.

Qualifications

• Must be a Licensed Practical Nurse or Registered Nurse.
• A minimum of one year of experience working with elders or disabled individuals, preferred.
• Must have demonstrated Leadership capabilities.
• Knowledge and experience in Assisted Living industry and Dementia care or Skilled Nursing, preferred.
• Ability to work weekends, evenings and flexible hours, available to our customers at peak service delivery days and times.

Responsibilities

• Monitors health and wellbeing of residents and supports wellness leadership.
• Medication care administration and communication with resident support services and families.
• Maintains clinical quality assurance while guiding wellness practices.
• Identifies ongoing needs and services of residents through PSP process.
• Ensures proper follow through and documentation for residents with changes in clinical care needs.
• Assists Wellness Director with completing monthly wellness visits and PSP to reflect monthly changes.
• Informs Wellness Director/Executive Director of changes in medication or service level.
• Assists with obtaining weights and vital signs monthly for each resident.
